PART 1
All artists must fill out the form on this page title LOUISVILLE WORK STATION PERMITTING and submit their information. Once submitted, click the link to pay the $100 Work Station Fee. We will submit the form and payment on your behalf.
PART 2
Each artist will need to be registered with Louisville Metro Department of Public Health and Wellness. If you already hold a LMDoPH artist permit, you can skip part 2.
Each artist will need to provide proof of blood-borne pathogen training.
If you need a new BBP Certificate, we recommend this one, which is OSHA certified.

Select only Tattoo Artist, as there is no piercing allowed at this event.
After providing your SSN, phone number, email, and address, you will select “yes” under Is This a Temporary Tattoo Studio
The start and end dates are 3/21/2025 – 3/23/2025

There will be a $100 fee paid directly to the LMDoPH.
